REMINDER … If you have not submitted your complete Immunization Record to include a Tetanus Booster (every ten years).
This is a STATE LAW that cannot be ignored if you are planning to remain on any NC college campus or participate in any sports. The deadline is tomorrow, September 27, 2024.
So, If you have received notice and have not submitted your complete immunization record or there has been no contact with the Health and Wellness office by Tuesday, October 1, 2024,
You WILL be removed from the campus for failure to comply with the State Law.
These are four simple steps in securing an Immunization report:
1. Have you contacted the Scotland Health Department to see if they can help you run an immunization report? The number is 910-277-2440.
2. Have you contacted your regular doctor at home to see if they can fax the record to Jacqueline Inman at 910-277-5061?
3. Have you contacted your high school to see if they can fax the record to 910-277-5061?
4. Have you contacted your parents/guardian to let them know that must have your shot record as soon as possible?
If assistance is needed, please reply to this email to schedule an appointment with me to discuss a course of action for the state law requirement as soon as possible.
