Reminder … If you have not submitted your immunization record.
This is a STATE LAW that cannot be ignored if you are planning to remain on any NC college campus or participate on any sports team.
These are four simple steps in securing an Immunization report:
1. Have you contacted the Scotland Health Department to see if they can help you run an immunization report? The number is 910-277-2440.
2. Have you contacted your regular doctor at home to see if they can fax the record to Jacqueline Inman at 910-277-5061?
3. Have you contacted your high school to see if they can fax the record to 910-277-5061?
4. Have you contacted your parents/guardian to let them know that must have your shot record as soon as possible?
If assistance is needed, please reply to this email to schedule an appointment with me to discuss a course of action for the state law requirement as soon as possible.
The Health and Wellness office is located in the DeTamble Library, 1st Floor near the Study Hall area.
